Our financial coordinators can answer your questions on insurance benefits, treatment fees, and the best alternatives for making treatment as affordable as possible.

Our financial coordinators will be pleased to address your questions on insurance coverage, obtaining maximum benefits, and out of pocket expenses. They can also help you determine the best alternatives for making treatment as affordable as possible and for managing treatment costs.

Our policy is to complete all of the financial arrangements before treatment begins, so we can focus solely on your clinical care once treatment starts.

Many of our first-time patients have questions regarding insurance coverage. We are happy to help you determine your coverage, including whether your first appointment is covered. Please call or email our New Patient Coordinator to request an appointment.

You will meet with a financial coordinator who will review your insurance benefits at the end of your new patient appointment. The resulting benefit verification is not a guarantee of coverage or payment. We strongly encourage you to confirm this benefit check yourself.

Most patients have coverage for some fertility treatments or diagnostics, whether 'in-network' or out of network'. We can help you determine your coverage and any special requirements your insurance provider may require, such as pre-authorizations.

At FSMG we have created over thirty different packages that discount diagnostic or treatment plans based on bundled services and payment prior to the initiation of therapy. Some of our more popular case rates are:

The ARC Family Building Program™

The ARC Family Building Program is a unique service and financial package that helps patients overcome insurance coverage limitations that prevent them from accessing the treatment they need. This program also gives you the confidence of a flexible reimbursement option refunding up to 100% of medical and pharmaceutical costs if treatment does not result in a live birth.

The ARC Family Building Program offers a number of affordable alternatives:

Three-Cycle Plus™ -- Maximizes your chances for a baby. Covers all the predetermined costs for up to three cycles of in vitro fertilization (IVF), plus up to three additional frozen embryo transfer cycles, for a total of up to 6 cycles.

Two-Cycle Plus™ -- Covers all the predetermined costs for up to two cycles of IVF, plus up to two additional frozen embryo cycles, for a total of up to 4 cycles.

One-Cycle Plus™ -- Your most affordable IVF choice. Covers all the predetermined costs for one cycle of IVF, plus up to one additional frozen embryo transfer cycle, for a total of up to 2 cycles.

The ARC Refund Program™ -- After receiving all of the required services you will be refundedmost of the money you have paid if a live birth (baby) has not been achieved through IVF treatment. The ARC Refund Program is available on the One-, Two- and Three-Cycle treatment packages as well as some Donor packages.

The ARC Pharmacy Plan™ -- Provides for all the predetermined costs of fertility medications needed for your care, which can be purchased separately or with any of the treatment options.
